Title V State Action Plans

As part of the Title V Maternal and Child Health (MCH) Block Grant, grantees develop a five-year State Action Plan to address their priority needs. This plan outlines both the grantee’s planned activities for the application year as well as the activities that were completed in the annual report year. Each plan is organized by the following six MCH population health domains:

  • Women/Maternal Health
  • Perinatal/Infant Health
  • Child Health
  • Children with Special Health Care Needs (CSHCN)
  • Adolescent Health
  • Cross-cutting or Life Course

In developing the five-year plan, grantees complete a Five-Year State Action Plan Table. This table includes the grantee's priority needs, program objectives, key strategies and relevant performance measures selected by the grantee for addressing their identified priority needs within each of the six population health domains.

In partnership with state Title V program leadership and other MCH stakeholders, MCHB identified 15 national priority areas for the Title V MCH program. Depending on a state or jurisdiction’s priority needs, Title V grantees chose eight out of 15 national measures to be addressed over the course of the five-year period in their Title V program.

Select a National Performance Measure priority area to see which states and jurisdictions chose it.
