Project Description

Approximately 23% of Ohio’s children between 0-17 years old have special health care needs, making the rate of CYSHCN in Ohio significantly higher than the national average of 19.4%. Data show that families of CYSHCN have difficulty accessing health care and receiving needed services. Transition into adulthood is particularly challenging and those with complex health care needs face challenges in accessing a medical home. Family members also report that they and professionals need to be trained on partnering and shared decision-making to improve relationships and health outcomes for children and youth with special healthcare needs.

Cradle Cincinnati Connections is dedicated to helping families thrive in Cincinnati. We connect moms-to-be with the services they need, filling system gaps that families fall through all too often. Cradle Cincinnati Connections is designed to enhance and support maternal and child health social support programs by streamlining the use of existing resources for families. Project Description

Project IMPACTT(Integrating Mobile Provider Access through Telemedicine and Transition) will increase access to coordinated, quality health care in a patient/family centered medical home for children and youth with epilepsy (CYE), many of whom may reside in rural and/or medically underserved areas. This will be accomplished by supporting quality improvement (QI) networks to address four content areas: (1) increasing access to specialists through telehealth and telemedicine strategies:(2) increasing family engagement at various levels across the health care system;(3) improving the transition from pediatric to adult health care; (4) increasing communication, collaboration, and co-management between primary care providers and epilepsy specialty providers.

Project Description

Universal screening for infant hearing is consistently implemented by Ohio hospitals prior to the newborn's discharge from the facility. However there is a continuing need for improvement in follow-up for infants who do not pass the hearing screening. Infants are lost-to-follow-up prior to receiving a diagnostic evaluation to confirm a hearing loss. Then, either during the diagnostic process, or after diagnosis of hearing loss is confirmed, additional infants are lost to follow-up prior to enrollment in early intervention. The Infant Hearing Program goals are to reduce losses to follow-up at both of these critical junctures through a variety of means including standards development, improved education of families and providers, collaboration with service providers and families, and outreach to educate and support families.

Project Description

The Ohio Title V agency is located within the Ohio Department of Health’s (ODH) Bureau of Maternal, Child, and Family Health (BMCFH). BMCFH’s primary goal is to serve as a safety net for all MCH populations including racial and ethnic groups disproportionately affected by poor health outcomes. To accomplish this, the agency requires expanded and enhanced Maternal and Child Health (MCH) data capacity that will support informed resource allocation decisions and outcome evaluation on an ongoing basis.
