Project Description

The Ohio Title V agency is located within the Ohio Department of Health’s (ODH) Bureau of Maternal, Child, and Family Health (BMCFH). BMCFH’s primary goal is to serve as a safety net for all MCH populations including racial and ethnic groups disproportionately affected by poor health outcomes. To accomplish this, the agency requires expanded and enhanced Maternal and Child Health (MCH) data capacity that will support informed resource allocation Decisions and outcome evaluation on an ongoing basis. The purpose of the Ohio State Systems Development Initiative (SSDI) is to expand and enhance current state and jurisdictional MCH data capacity and to develop new more timely data systems and infrastructure that will support MCH program objectives in alignment with the Health Resources and Services Administration (HRSA) Title V block grant.

Project Description

Unintentional injury is the leading cause of morbidity and mortality among children in the U.S. after the first year of life. Children under 6 years of age spend much of their time at home and therefore a large proportion of all injuries in this age group happen at home. Injuries in the home accounted for 75% of all injury-related emergency department visits at Nationwide Children’s Hospital among children younger than 6 years old in 2010. Children need safe environments where they can challenge themselves and develop physically, cognitively and emotionally. The home environment was designed by adults for the convenience of adults, and child safety has often been an after-thought. This project will employ a multi-faceted, community-based approach to prevent these injuries among children under the age of 6 through outreach activities aimed at pediatricians, public health agencies, and other agencies and organizations that work with children.

Cradle Cincinnati Connections is dedicated to helping families thrive in Cincinnati. We connect moms-to-be with the services they need, filling system gaps that families fall through all too often. Cradle Cincinnati Connections is designed to enhance and support maternal and child health social support programs by streamlining the use of existing resources for families. Additionally, Cradle Cincinnati Connections directly serves families and children by providing access to a team that includes a nurse, social workers, community health workers, a dietitian and a mental health specialist.

Project Description

Universal screening for infant hearing is consistently implemented by Ohio hospitals prior to the newborn's discharge from the facility. However there is a need for improvement in follow-up for infants who do not pass the hearing screening. Infants are lost-to-follow-up prior to receiving a diagnostic evaluation to confirm a hearing loss. Then, either during the diagnostic process, or after diagnosis of hearing loss is confirmed, additional infants are lost to follow-up prior to enrollment in early intervention. The Infant Hearing Program goals are to reduce losses to follow-up at both of these critical junctures through a variety of means including standards development, improved education of families and providers, collaboration with service providers and families, and outreach to educate and support families.

Project Description

Approximately 23% of Ohio’s children between 0-17 years old have special health care needs, making the rate of CYSHCN in Ohio significantly higher than the national average of 19.4%. Data show that families of CYSHCN have difficulty accessing health care, receiving services they need to transition into adulthood and that those with complex care needs face challenges in accessing a medical home. Family members also report that they and professionals need to be trained on partnering and shared decision-making to improve relationships and health outcomes for children and youth with special healthcare needs.

Project Description

PROBLEM: Disparities persist in the availability and quality of health services for individuals with neurodevelopmental disabilities (NDD) and their families depending on income, racial/ethnic background and geographic location. These disparities can be addressed through innovative approaches to health care systems and through training in leadership, scholarship and community partnerships which impact health care and community systems.
